Dear colleagues,
We have fulfilled the 10th Congress of Trauma & Emergency Surgery between 13-17
May, 2009 in Antalya, Turkiye. The congress was organized with the collaboration
of the Trauma & Emergency Surgery Society (ESTES) and the Turkish Association
for Trauma & Emergency Surgery Society (TATES). The Turkish Orthopedic
Association (TOTBID) and the European Federation of National Associations of
Orthopedics and Traumatology (EFORT) also actively supported the ESTES 09
meeting.
The 10th Congress of Trauma & Emergency Surgery was attended by 1,306
participants from 55 countries and from six continents (Table 1). Sixty percent
of the participants were from Turkiye and 40 % were from abroad.

A total of 1,054 abstracts were submitted for the ESTES 09 meeting and 806 (76 %) were accepted. Of these abstracts 347 were presented as oral presentation, 453 as posters and 6 as video presentations. The meeting was supported by 216 speakers; 149 of which were from Turkiye and 67 were international (Table 2).

A total of 45 scientific sessions in four different rooms were performed: 13 in
Emergency Surgery, 12 in Visceral Trauma, 12 in Skeletal Trauma and 8 in
Disaster, Military Surgery & Pre-hospital management. Four pre-congress courses
on Laparoscopy, Wound Care, Pre-hospital Mass Casualties, and Surgical
infections were fulfilled simultaneously.
I would like to thank the board members of the mentioned societies, our national
and international colleagues, ESTES institutional and individual members and our industrious Symcon PCO partner to make this
event a really successful one. We look forward to the success of the Congress of
Trauma & Emergency Surgery between 15-18 May, 2010 in Brussels, Belgium.

In an earlier post, I
indicated that I attended and was invited to be a speaker at the European
Society for Trauma and Emergency Surgery (ESTES) in Antalya, Turkey. This was a
spectacular meeting with 1200 attendees, of trauma surgeons, critical care
surgeons, orthopedic surgeons, thoracic surgeons, abdominal surgeons, vascular
surgeons, foregut surgeons, minimally invasive surgeons, surgical intensivists,
trauma resuscitatiologists, anesthesiologists, emergency physicians, acute care
surgeons, emergency surgeons, and trauma/emergency medicine public health
physicians. The meeting was in two different large rooms with those interested
in head, neck, thoracic, abdominal, and vascular surgery in one room, and those
with primarily osseous and soft tissue extremity injury meeting in the other. In
addition, medical students, residents, fellows, registrars, administrators,
CEOs, CFOs, COOs, and public policy political persons were present.
The comments in this
post are totally and solely my own. NO ONE has suggested I write this editorial,
and no one has reviewed anything I have written here.
Many of the above
medical professional persons are listed due to the NON STANDARDIZED nomenclature
of physicians in Europe and the Middle East. Around the world, but especially in
EUROPE there is a great need for a standardization of nomenclature as to what
physicians are called. ESTES is in an excellent position to accomplish this long
needed nomenclature
standardization. More
importantly, there is great debate as to just what each named "specialist" can
do in the EMS, resuscitation area of the hospital, dispatch, disaster, OR, ICU
and clinic locations. Many qualified physicians
are literally
prohibited from caring for patients because of this jealous
protection of turf,
both in Europe and in the United States. ESTES is also in the most ideal
position to address these turf and credentialing
challenge. THIS MUST BE
DONE if there is to be progress in the care of emergency surgery patients, be it
in disaster, burns, interpersonal trauma, war, industrial accidents, road
traffic accidents, or others. Furthermore, the
systematic public
health approach to trauma care is going to be mandated by
the consumerism
movements of governments and the public in general. I was struck with the
protectiveism of the word "trauma" and just who owned it. In each country,
different specialist tended to want to "own" the term "trauma" for their
discipline. No where did this seem to be more volatile than in Germany where the
orthopedist who does almost exclusively bones, joints, and soft tissue wanted to
continue to solely own the term trauma. This may be an over simplification on my
understanding, but it seemed to me that if the German orthopedic surgeons would
merely call themselves orthopedic surgeons with interenst in "______" (pick any
term), much progress would be made in Europe.
It is time that all of
us address a trauma and emergency surgery Integrated
Collaborative Network
with a public health focus on our approach. This means a system approach. I am
looking forward to ESTES leading the entire world on their approach to these
issues. There was much discussion in formal meetings and from panels on these
subjects, but no closure or total agreement. I also found a significant
disparity between what was said by the "elder" trauma and emergency surgery
statesmen (and officers of the organization) and the younger less vocal surgeons
from each country. I noted none of the YOUNG persons at the table during formal
panels. I also did not note ANY officers who were female. Much progress will be
made by ESTES in the future and I do hope to be an observer of these advances.
The whole world will benefit from such an ESTES leadership.

Geçtiğimiz hafta yapılan 10. Avrupa Travma ve Acil Cerrahi Kongresi hakkındaki
izlenimlerimi paylaşmak istedim:
- Böyle yoğun ve çeşitli branşlardan katılımlı bir uluslararası kongreyi
organize eden ve hiçbir aksaklık olmadan sonuçlandıran, başta Prof. Dr.
Korhan Taviloğlu olmak üzere emeği geçen herkesi kutlarım.
- Dr. Mehmet Eryılmaz "PREHOSPITAL MASS CASUALTY EXERCISE & TRAUMA
MANAGEMENT COURSE"da harikalar yarattı. Özellikle Diyarbakır bombalamasının
simüle edildiği tatbikat ortamı çok başarılıydı. Yabancı katılımcıların da
çok etkilendiklerine şahit oldum. Sayın Eryılmaz'a ellerine sağlık demek
gerekiyor.
- Kongre katılımı ile ilgili bir sıkıntı kongreye kayıtlı olduğu halde,
ucuz dış otelleri tercih eden özellikle asistan meslektaşlarımızdan günlük
50 avro gibi fahiş bir ayakbastı parasının tahsil edilmesi oldu. Sırf bu
nedenle katılımını sınırlandırmak zorunda kalan asistanların varlığı üzücü.
Gittikçe fahiş fiyatlara yükselen kongre kayıt ve konaklama ücretlerinin
bizleri hem kendimiz hem asistan arkadaşlarımız için firmalarla bu kadar
diyalog kurmaya itmesi beni kişisel olarak çok rahatsız ediyor.
- Çok büyük isimler katıldı kongreye. Mattox, Asensio, Ivatury,
Demetriades gibi. Belki beklentilerimiz fazla olduğundan, konuşmalarında
beklediğim tatmini aldım dersem yalan söylemiş olurum. Bu isimler, özellikle
de Mattox, kendilerini literatür üstü gördüklerinden olsa gerek, artık
konuşmalarını çok fazla kendi doğruları üzerinde yapıyor. Boyun, toraks
travmalarının konservatif takibi gibi önemli bir konuda pek de fazla birşey
anlatılmadı hissi ile çıktım salondan. ya da Mattox'un "Assesment and
management of intravascular volume" gibi bir konuda, buna hiç değinmeden
günlük pratiğini anlatması garip geldi bana. Hasar kontrol resüsitasyonu
gibi yeni gelişen bir kavrama bir cümle ile değinip son derece didaktik
"benim dediğim doğrudur" vurgusu ile bir takım önerilerde bulundu. Transport
süresi 3-5 dk olan ve oturmuş travma merkezlerinde, sadece penetran travmalı
hastalarda 15 yıl önce yaptıkları ve pek çok kritik almış bir çalışma
üzerinden literatür hakkında hiç konuşmadan kesin öneriler verdi. Onların da
hiç bir ayrıntısına girmedi. halbuki resüsitasyonda değişen kavramlarla
ilgili, asistanların çok faydalanabileceği bir konuşma bekliyordum ben "Top
knife" gibi travma cerrahisinin felsefesini yapan bir kitap yazmış olan bir
yazardan. Aynı şekilde Ivatury'nin pankreatikoduodenla travmalar,
Asensio'nun abdominal vasküler yaralanmalar konuşmalarını da biraz buruk
dinledim. Oturum sonunda asistanıma "travma hastasında hangi damarları gözü
kapalı bağlarsın, vasküler hasar kontrol cerrahisini nasıl yaparsın ?" diye
sorduğumda hiç bir cevap alamadım çünkü anlatılmadı. Ya onlar hafife aldı
konuları, yada benim beklentilerim fazla idi. Okuduğumuz, yaptığımız,
uyguladığımız, bildiğimiz şeyler hakkında daha ciddi konuşmalar bekledim
belki. Bu toplantılara katılan bir son sene asistanı ya da yeni bir uzmanın
çok faydalanmış olacağını düşünmüyorum kendi adıma.
Genel olarak çok başarılı geçen kongre için emek harcayan herkese tekrar
teşekkürler,

ESTES kongresi bir harikaydı. Ülkemizde böyle bir kongrenin düzenlenmiş
olmasından büyük onur duydum, keyif aldım. "Emeği geçenler" demek yeterli
gelmiyor bana. Korhan Taviloğlu'nun her aşamasındaki tartışılmaz emekleri bu
kongreyi yarattı. Symcon yine çok iyiydi. Ancak yine kongrenin parasal yönü
dillerde konuşuldu. 70 Avro gala yemeği, günlük otele giriş ücretleri vs... Ne
olur biraz daha ekonomik organizasyonlar yapabilsek, asistanlarımız daha çok
katılabilse. Hepimizin olan bu para çok daha doğrudan eğitime aktarılabilse...
Rixos'un cazibesi, havuzlar, yemekler, aktiviteler salona giren yabancıların
akıllarının dışarıda kalmasına neden oldu. Bazı yabancılar doğrudan salon dışını
tercih ettiler. Hersey cok güzeldi.
İlgi alanım gereği diger bilim dalları ile ortak çalışmalar yapıyorum. ULUSAL
TRAVMA VE ACIL CERRAHı DERNEĞİ şu ana kadar yaptıkları ile Türkiye'deki hemen
bütün derneklerinden daha iyi çalışmıştır. Ana Derneğimiz Türk Cerrahi
Derneği'nin yakın zamana kadarki konumundan dahi daha etkin olmuştur. (Türk
Cerrahi derneği'nin şu anki çalışma temposuna şapka çıkarmak gerekir).Travma
Derneği'nin bu başarısı nedendir? Kanımca bu başarının nedeni yönetim kurulu
içerisindeki var olan sinerjidir. Her biri farklı yapıda sayılabilecek öğretim
üyeleri bir araya gelmiş başka hiçbir kaygı duymadan ülkedeki travma ve acil
cerrahi konusuna el atmıştır. Beğenelim veya beğenmeyelim şu anki sağlık
bakanlığı yönetimi ile ilişki kurulmuş TRK Ulusal bir kurs haline gelmiştir.
TRK'yı yapmak önemlidir. Ancak bunu resmi kurumlara kabul ettirmek de önemlidir.
Dergi SCI'ye girmiştir. Bekleme süresi, kabul ret oranları ortadadır. Bu dergiyi
ortaya çıkarmak önemlidir. Ancak bu dergiyi gündemde tutmak, engel olmamak,
köstek olmamak da önemlidir. Alanında ilk (bazı kötü benzer başlıklı kitaplar
olmakla birlikde) iki kitap basılmıştır. Travma ve Acil cerrahi kitapları... Bu
kitaplar önemlidir. Türk bilimine, Hekimlik mesleğine büyük katkılar
sağlanmıştır.
Bizlerin görevi Bu derneğin yaptıklarına destek olmaktır. Bu noktada da büyük
emekler söz konusudur. Ankara'da TRK yıllardan beri yapılagelmiştir. Zincirin
halkalarında zaman zaman bazı zayıflıklar olsada hiçbir zaman bu durum işin
sürekliliğini ve düzenini sekteye uğratmamıştır. Benzer şekilde sanırım tüm
Türkiye'de Derneğe aynı destek soz konusudur.
Dileğimiz özellikle dernek yönetim kurulundaki bu sinerjinin sürekli olmasıdır.
Çünkü derneğin ve biz dernek periferinde calışan ve derneğe koşulsuz destek
veren hekimlerin yaptıkları tümüyle bu sinerjiye destek adınadır. Yaklaşan
dernek genel kurulu toplantısında bu noktanın gözardı edilmeyeceğine olan
inancım her zamanki gibi tamdır. Ortada yılların emekleri ile yaratılmış bir
değerler bütünü vardır. Bu değeri korumak hepimizin görevidir.
Yazımı bitiriken herkese iyi bir haftasonu diliyorum. Tekrar Dernek başkanı Dr.
Ertekin'i ve Kongre başkanı Dr. Taviloğlu'nu başarılarından dolayı kutluyorum.

Bir parçası olduğumu hisssettiğim ve bundan çogunlukla güven ve mutluluk
duyduğum derneğimizin de desteğiyle geçtiğimiz günlerde yapılan ESTES 2009 un
sorunsuz ve üst düzeyde bir süreçle tamamlanmasından büyük gurur duydum..
Cerrahiyi sevmemin temel nedeni ekip işi oluşudur ama ekip onu dogru yöne
götürecek bir lider varsa başarıyı yakalar.. işte ESTES de bu biçimde iyi
başladı iyi devam etti ve çok iyi izler bırakarak tamamlandı..Bu kongrede
herkesin az çok emeği var ve hepsi çok değerli , ama bence aslan payı kongre
başkanı olan değerli arkadaşımız Korhan ın., .evet sevgili Korhan sağol varol
emegine ve yüreğine sağlık..bizi hem onurlandırdın hem gururlandırdın...hem de
